Physicians and other
health care providers have used health assessments for over
20 years to educate patients on potential health
risks that are related to lifestyle and family history.
Initially patients completed assessments on paper.
Physicians then scored the assessments and discussed the
results with patients.
Unfortunately,
due to the cost pressures and time constraints healthcare
providers face today, most can no longer afford to provide
their patients with a complete health assessment.
Our assessments and
clinical reports
offer you a unique opportunity to obtain important
information about your risk of prescription drug-nutrient
interactions, drug-induced nutrient depletions, symptoms of
nutritional imbalances, or if your current meal plan
supplies enough nutrients. When ordered, our
nurses will lead you through a series of questions that will
help determine your individual health risk. At the end of
the questionnaire you will receive a customized report that
will tell you about your health risks. You may wish to
discuss making changes with you family doctor or local
health care provider so that he or she may make further
recommendations based on the assessment results.
Reports
Our reports have
more than 50 pages of detailed information about 30
different nutrients and how they may relate to your
current level of health. Our assessment reports are
mailed to you directly. These reports are an important
clinical tool for your doctor in deciding if lab testing is
an important next step in your care plan, to include meal
plan changes, or nutritional supplements in your overall
care regimen.

through the process you will discover health factors that

may need more attention based on the information you provide

helping your employer and health plan determine what types
of programs need to be implemented to help you improve your
overall health.

your health. Remember as you complete the assessment
questions that your results will depend on how accurately
and completely you answer the survey. Before you take
any action based on the report information, you should
discuss the results with your health care provider.
The privacy of
your health information is a key concern
for all healthcare providers, including your employer and
health insurance plan. We have also take this responsibility
very seriously and have taken steps to ensure that your
health information will be protected and kept private and
confidential.
Access to information collected by nurses
is unauthorized and not available to any private parties.
Information collected in your
assessments are used in two ways.

First, all of the data collected is combined and
may used by
your employer and/or health insurance plan to determine
the overall health of the employees. This aggregate
information
does not
contain information about individual employees, but does
shows the major nutritional risk factors for the entire
group. |

Employers and/or health insurance plans
may analyze this
information to help make decisions on what programs need
to be offered to employees to help improve their general
health and to lower healthcare costs. |
