Insurance:
Insurance and Payment Policy
Please consult your insurance provider for a list of preferred providers. Co-pays and deductibles are due at the time of service, as required by your insurance company. If you do not present your insurance card and payment at check-in, your appointment may be rescheduled, or you may be responsible for the full cost of services.
If your insurance requires a referral, please obtain it in advance to avoid appointment delays.
Optimal Health Associates may attempt to pre-certify certain radiologic tests; however, not all imaging or lab tests can be pre-certified. Lab tests ordered by your provider will not be pre-certified. We proceed with medically necessary evaluations regardless of insurance coverage. It is your responsibility to understand your insurance benefits for lab work, radiology, and other services, as coverage varies and may change frequently.
We welcome self-pay patients. Please, contact our office to discuss fees.
For billing questions, contact our office at (405) 715-4496 or send a message in your Healow portal. You can pay your bill directly in your portal as well.
Initial Visit
Your initial consultation includes a review of your medical history and medications, physical examination, review of prior laboratory tests, and an overall assessment of your healthcare needs.
Any procedure or surgery to be performed will be discussed with you and scheduled for another date. Our staff will gladly answer any questions you may have about your office visit and recommendations.

Refills will not be processed after hours.
Controlled Substances
Medications such as Adderall or other controlled substances require a 3-day notice for refills and cannot be called in. These prescriptions are monitored closely by your provider. Patients are responsible for managing their controlled medications and should address any concerns during scheduled appointments.

For all refill needs, please contact your pharmacy directly or send a message through the Healow app. Your provider or nurse will respond within 48–72 hours.
